Our quick intake process for new patients helps us gather everything needed to confirm your appointment and prepare for your care.
It typically takes 5 minutes if you have these things ready:
- Photos of your government-issued ID and the front and back of your insurance card (if using)
- A list of medications, conditions, and allergies
- Your pharmacy’s name and address
- Your preferred credit card to keep on file
Doing this now means no surprises later — just reliable scheduling, upfront billing information, and a provider who’s prepared to start your care plan from day one.
If you’re an existing patient, don’t forget that you can log in to the patient portal to manage and book appointments.
