Who's Eligible & How It Works
New patients booking their first appointment with Brenda Donovan or Bryan Reddick are eligible.
Your appointment must be booked during the promotion period, between now and July 3, 2025. One entry per person.
Three winners will be selected after the promotional period ends on July 3, 2025.

Eligibility
This promotion is open to new patients who are legal residents of the United States, reside in Arizona or Colorado, and are 18 years of age or older at the time of entry. To be eligible, participants must book their first psychiatric appointment with one of the following Peak Mental Healthcare psychiatric nurse practitioners: Brenda Donovan or Bryan Reddick. Existing patients of Peak Mental Healthcare are not eligible.
How to Enter
Eligible participants will be automatically entered into the giveaway upon completion of booking an appointment with Brenda Donovan or Bryan Reddick via Peak Mental Healthcare’s official booking system during the promotional period, which begins on [start date] and ends on July 3, 2025 (“Promotion Period”).
Limit one (1) entry per person.
Prize Description
Three (3) winners will be selected following the end of the Promotion Period. Each winner will receive one (1) gift item valued at $100 or less, selected by the winner from a curated list of available items provided by Peak Mental Healthcare. No cash or gift card substitutions will be permitted. Prize is non-transferable and subject to availability.
Peak Mental Healthcare reserves the right to substitute a prize of equal or greater value in the event the selected item becomes unavailable.
Winner Selection and Notification
Winners will be randomly selected on July 8, 2025, from all eligible entries received during the Promotion Period. Winners will be notified via the email or phone number provided at time of booking. If a selected winner cannot be contacted within 7 days or is determined to be ineligible, an alternate winner may be selected.
General Terms
By entering, participants agree to be bound by these Official Rules and the decisions of Peak Mental Healthcare, which are final and binding in all respects. Peak Mental Healthcare reserves the right to disqualify any participant who violates these rules or tampers with the operation of the giveaway.
This promotion is subject to all applicable federal, state, and local laws and regulations. Void where prohibited.
